A student must complete their last course through Indiana Tech. The grade earned must be a “C-” or higher for transfer credit to be accepted for undergraduate degrees. Student must have completed the pre-requisite requirement for the course requested on this form. If the student was enrolled in the course to which this form applies and withdrew, the student cannot take the course off campus. SACM students cannot apply if course is offered within same semester of request at Indiana Tech. SACM students cannot take online courses or hybrid courses. Upon completion of this course, an Official Transcript must be submitted for transfer credit to be reviewed and applied (if applicable).
